Forms Admin
The Forms module allows organizations to create and manage digital forms for special requests and communications with users. Its primary purpose is to streamline data collection and digitize specialized workflows directly within the system. This article is intended for OPSCOMOPS-COM administrators whoresponsible needfor to setsetting up, configure,configuring, and managemanaging these digital forms.
Setup &and Configuration
Administrators must ensure they have the appropriate role permissions assigned before attempting to create or modify forms.
Admin Side: Administrators require specific permissions to access the form builder tools.

Hover over System Configuration, click Admin Management, and click Manage Roles.

Select the relevant administrator role and click the Permissions button.

Navigate to the Users tab and enable the Edit Forms and View Forms checkboxes.

Click the Save button to apply the changes.
The Forms module has been refactored with new functionalityfunctionality, and new forms are currently only operational on the OPSCOMOPS-COM app.app. Any existing forms created in the old format are not compatible and will need tomust be recreated using the new form builder.
Using this Feature
ToAdministrators managecan forms,use administratorsthe requirefollowing specificinstructions permissions:to access the module and utilize the available form management tools.

GoHovertoSystem Configuration,thenAdmin Management,and clickManage Roles. Select the relevant administrator role and clickPermissions.Under theUserstab, ensure theEdit FormsandView Formscheckboxes are enabled.ClickSave.
Using this Feature
Administrators can view, edit, or create new forms from the Forms page.
Accessing the Forms Module
Go toover User Management,Management, hover over Forms, and click List Forms. to Aview the complete list of all existing forms will display.forms.
Available Actions &and Buttons
Once on the list page, administrators can interact with forms using the following actions:

+Add Form:
To create a new form, clickClick the +Add Formbutton.buttonThis willto open the new form builder, allowing you to design and configure your form from scratch. -
Edit
Form:To modify an existing form, clickClick the Edit icon (represented by a pencilicon)icon) next tothea form in thelist.listThis willto open theformbuilderwhere you canand makechanges.changes to the layout or questions. -
Entries:Entries: Click the EntriesTobutton to view a comprehensive list of all users who have filled outthisthe specific form,andalong with their submitted information. -
Copy:
ToClickcreatethe Copy button to immediately duplicate acopyform.ofTheacopiedform,versionclickwillthis button. It isbe added to the bottom of the list. -
Archive:
ToClickarchivethe Archive button to retire a form so it can no longer beused,usedclickbythisend-users.button.AllItspast entries and submitted information will remainsaved.saved for historical reference. -
Export:
ThisClick the Export buttondownloadsto download the form data to your device throughtheyour browser's download manager in a .xls format (Microsoft ExcelSpreadSpreadsheet).Sheet)
Best Practices &and Considerations

Recreate
OldlegacyFormsforms promptly::Remember that existing legacy form data is not compatible with the new format. You must actively recreate any old forms using the new form builder for them to function correctly on theOPSCOMOPS-COM app. -
PermissionsAudit permissions regularly::Regularly review and ensure that the appropriate administrator roles have the necessary permissions.
